Description

In the United States, more than 40M adults are affected by diabetes, 20M have been diagnosed with cardiovascular disease (CVD) and another 37M are estimated to have chronic kidney disease (CKD). A whopping 138M adults are obese. There is only 1 endocrinologist for every 50K people diagnosed with diabetes, 1 nephrologist for every 30K CKD diagnosis and 1 cardiovascular specialist for every 15K CVD diagnosis. The average wait time to see these specialists across the county is often over 6 months.

Broadly adopting new methodologies of chronic condition management support in most health plans takes considerable time. But the pandemic rapidly forced many plans to implement and take a leap of faith in supplementary, virtual, clinical support models.  

This session will review how UCare built a holistic care management methodology that supports each type of member, regardless of tiered risk level status or preferred methodology of engagement. The session will also discuss UCare's internal care management strategy; cover where the vendor market has been, and where it is heading; the pros and cons of different strategies; how to measure outcomes honestly and fairly; and what results can be reasonably expected. The session will detail outcomes achieved across programs: what to look for, what to avoid and how to measure metrics beyond fundamental and required quality measures. It will walk through member stories, case study results and how these programs empowered members to live their healthiest and happiest life.
