Medstar Health set out to transform an isolated risk calculator into an open standards-based application that seamlessly and securely runs across health IT platforms and provider organizations to optimize clinical workflow, improve care coordination and support patient engagement. Specifically, a SMART on FHIR app called “Mobilizing a Million Hearts,” which integrates the Million Hearts Longitudinal Atherosclerotic Cardiovascular Disease (ASCVD) risk calculator into the MedStar Health EHR system.

MedStar won the ONC LEAP grant in 2018 to develop this application using standards-based integration and open source software, then turned the demonstration into a long-term plan to keep the successful app in workflow.
