Commonwealth Care Alliance (CCA) will present its successful digital quality initiatives that have improved workflow efficiencies and health care outcomes, and will share insights into driving digital quality by leveraging technology, including robotic process automation (RPA) and data cleansing, to improve data integrity and completeness.

To improve post-discharge workflows, data capture of medication reconciliation post-discharge and care for members during a care transition, CCA used RPA and automated admission data notifications to reduce administrative burden for the care team. An RPA process was initiated to upload discharge instructions using a uniform methodology accessible to all staff.  

CCA also launched a pilot program to update and clarify the post-discharge medication reconciliation workflow of RN Care Partners and Community RNs. This cross-collaboration between clinical and quality teams improved access, compliance and quality standards. 

In addition to these initiatives, CCA leveraged data cleansing tools to improve the quality and completeness of data obtained in a designated field in the legal health record. Collaboration with Alt+eryx created a data cleaner tool that transforms free text fields into date fields with over 95% accuracy.
