Description

Tele Critical Care (TCC) is a population management approach to critical care that leverages a remote monitoring software system and audio-visual communication. TCC collaborates with bedside clinicians to care for ICU patients through proactive workflows, quality and safety checks, triaging and responding to requests from bedside clinicians, patients and family members, as well as assisting with emergencies.

The TCC team developed a standardized tool for communication, Hub Task Management (HTM), which allows prioritization of calls, assigning work by team, tracking workload and consultation care provided, in addition to complete data storage. 

HTM reporting capabilities include a complete dashboard display of calls received, time and duration of the call, call prioritization and type, medical facility calling, designee of person calling, in addition to the patient information. Reports can be viewed by day, week, month or year. Staffing workload can be evaluated from data retrieved. 
HTM has allowed Northern California Kaiser Permanente TCC to expand: it is in 12 of 21 medical centers, with plans to expand to the remaining 9 within the next year.
