Description

Phreesia is a health care technology company that helps medical groups and health systems automate and manage patient intake to achieve their strategic objectives and improve health outcomes. Phreesia’s innovative technology helps facilitate more than 100 million patient visits annually through a robust platform of applications that not only includes core services such as patient registration, and clinical support, but also patient activation. 

Higher levels of patient activation have repeatedly been shown to be associated with better patient clinical outcomes, mental health outcomes, medication adherence, disease self-management and treatment satisfaction across a number of clinical specialties and their correlating patient populations and chronic disease states. Improved patient outcomes and experiences lead to decreased costs and provider burden because activated patients require less clinical intervention.

This session will discuss the PAM, a brief survey that assesses patients' ability to manage their care, its application across different stakeholder groups and how it can be used to strengthen clinician-patient relationships and improve clinical outcomes.
