The path to a healthy life starts with people having access to nutritious food, available transportation, stable housing, employment and economic security and support from their families and communities. Decades of research show that social drivers can determine up to 80% of health. Achieving health equity involves addressing social determinants of health and health disparities. Success is decreasing disparities in outcomes among populations.

Elevance Health is taking steps to advance health equity through an integrated, whole-health approach. Digital transformation is partly responsible for making this possible. Seeing the person first, rather than seeing only their health challenges, will allow us to transform the health care system by putting the whole health of people at the center.

This presentation illustrates how Elevance Health integrates disparate data to provide analytics and insights that promote informed decisions about interventions. It will showcase the work of health plans that use holistic data to understand the needs of membership and their communities' social risks, identify populations with the greatest health disparities and stratify groups for targeted interventions, collaborate with key stakeholders to address members’ clinical conditions and social needs, measure key performance indicators to track intervention outcomes and share learnings to scale best practices across the enterprise. This whole-health framework supports health plans in meeting NCQA Accreditation standards for Population Health Management, Health Equity and Health Equity Plus.
