Description

Value-based care is an incentivized health care payment model where better patient care leads to higher reimbursement rates. Care depends on high-quality message exchange, for immediate clinical use, to ensure the most informed treatment plans.

Through meaningful engagement, the Veterans Health Information Exchange Clinical Data Quality team has fostered dozens of health care system relationships, expanding each year. These collaborations offer participating external partners a platform to review the C-CDA documents exchanged in the health care ecosystem.

Initial participating external partner/provider collaboration develops the core building blocks for cultivating a relationship and offers a baseline for clinical data quality. The continuation of this relationship establishes rapport, strengthens integrity and drives clinical data quality improvements that have downstream impacts on interoperability and value-based care.

The results discussed in this presentation aim to demonstrate the importance of positive engagement, leading to improved clinical data quality exchanges using C-CDA and areas of opportunity when developing FHIR standards.
